To make an appointment with Drs Eisenberg, Jimenez or Piorkowski, please call our office in advance to schedule an appointment. If you’re unable to keep your appointment, please call us as far in advance as possible so that we may reschedule that time slot.

Fees and Payments

The office of Drs Eisenberg, Jimenez and Piorkowski requests payment arrangements for all services at the time they are rendered unless prior arrangements have been made. We will bill your insurance company: however, payment is the patient’s responsibility. We’ll help in every way we can to assist handling claims.
